NT-proBNP & Heart Disease: Interpretation Guide¶
NT-proBNP is a cardiac peptide released when the heart is stretched or overloaded. It is the key POCT marker for distinguishing cardiac from respiratory causes of dyspnea and for staging heart failure.
The Core Clinical Question: Cardiac or Respiratory?¶
A dyspneic patient presents a fork in the road: cardiac disease or respiratory disease? NT-proBNP answers it quickly:

Species-Specific Cutoffs¶

Note the 9× species difference — feline and canine cutoffs are not interchangeable.

- Dyspnea triage — cardiac vs respiratory in minutes.
- Occult disease screening — breed-screening (e.g., Doberman, Boxer) before clinical signs.
- Treatment monitoring — trending NT-proBNP tracks response to heart-failure therapy.
FAQ¶
Can NT-proBNP diagnose the specific heart disease? No — it indicates cardiac involvement/stretch, not the specific disease. Echocardiography identifies the underlying disease.
Does renal disease affect NT-proBNP? Yes — reduced GFR can elevate NT-proBNP. Interpret in context of renal function.
Case Study: The Dyspneic Spaniel¶
A 10-year-old Cavalier King Charles Spaniel presents with acute dyspnea and a cough. Thoracic auscultation is difficult due to referred upper-airway sounds.
- NT-proBNP: 2,150 pmol/L (markedly elevated; reference < 900).
Interpretation: The elevated NT-proBNP shifts the differential toward a cardiac cause. Echocardiography confirmed myxomatous mitral valve disease with congestive heart failure; diuretic therapy was started, and NT-proBNP trended down with treatment.
